Payment

  • Level 5 does not accept insurance payments

  • We are not contracted with Medicare or MediCal

  • If requested, a "superbill" will be provided for patients to submit to insurance for possible reimbursement

  • Before you decide to start treatment with Level 5, we can provide you with a Good Faith Estimate of your costs:

    • if you were to receive no insurance reimbursement, or

    • if you were to decline to contact your insurer, or 

    • if you are uninsured

  • Convenient payment of fees by credit card through patient portal
