In-home or facility visits to elderly patients with cognitive and mobility challenges, on a case-by-case basis
Frequency of visits varies according to patient needs and MD availability
Weekend and evening visits by special arrangement
Convenient, confidential communication using secure messaging through password protected patient portal
To protect your privacy, Level 5 does not use conventional email for confidential medical information.

Currently the Level 5 practice is limited to patients physically located in California
